Apple A2M6021 (825-1352-A). This is the platinum colored monitor to match the extended, platinum IIe. The earlier beige model is found here.

This monitor works great. There is medium discoloration you can see from a lighter areas from a former sticker on the back of the top housing. The top right side of the bezel is broken. There are numerous scuffs, scrapes, dings and scratches on the case and it is missing the front control panel cover.

First photo is not this monitor, but the rest are.

No card required, just plugs into the video jack of any Apple II with included RCA type monitor cable. Power cable is attached and ships with RCA cable.
